The Pre-Planned Absence Form is used to inform directors of an anticipated absence (i.e., vacations, conflicts, etc.)
The Pre-Planned Absence Form must be completed and TURNED IN TO MR. STOWE at least two (2) weeks prior to any missed performances or at least one (1) week prior to any missed rehearsal. Emergency absences (i.e., illness, injury, etc.) must be communicated to Mr. Stowe at the next scheduled band meeting or practice, otherwise the absence will be considered unexcused.
The Volunteer Application and Volunteer Code of Conduct are used to record contact information pursuant to school policy.
The Volunteer Application and Volunteer Code of Conduct MUST BE COMPLETED EACH YEAR (even if you have a Level 2 clearance and your background check is still valid).
There are two levels of "volunteer" as defined in form. Level 2 volunteers must undergo a background check and pay the fees associated with doing so. The background check is valid for 5 years.
The Volunteer Application and Volunteer Code of Conduct forms should be TURNED IN TO EITHER: (A) A BOOSTER CLUB OFFICER; OR (B) THE SCHOOL OFFICE.
The Financial Hardship Form is used to inform the Director and Booster Club Executive Board of a need for financial assistance with booster fees. It does not apply to the school district's pay-to-play fee - that goes through the school directly.
The Financial Hardship form should be TURNED IN TO EITHER: (A) MR. STOWE; OR (B) A BOOSTER CLUB OFFICER.
